Friends and Family Test

The Friends and Family Test has been introduced to give patients the opportunity to provide feedback about the care and treatment they have received from the Practice.

It is a short questionnaire asking whether or not you would recommend our Practice to your family and friends, if they need similar treatment or care.

It is a quick and easy way of collecting patient feedback to help us celebrate success, focus improvements and tackle poor quality care, by answering a simple question about your experience.

Please download the Friends and Family Test here and either email it to us at feltwell.surgery@nhs.net or hand it in to us the next time your visit the Practice.
