How to Register

New Patients

  We can only register patients who live within our Practice area.
  To register with the Surgery, please ask at reception and they will give you a New Patient Registration form and a Health Questionnaire to complete.
  Two forms of ID are required to register with us, one must be photo ID (Passport, Driving Licence) and one must be a current utility bill (within the last 3 months).
  All new patients are required to attend a New Patient Medical with our HCA. Please help us by attending this appointment as it enables us to get your up-to-date medical history whilst waiting for your notes from your previous Practice to arrive (this process can take up to 3 months).

We are sorry we do not currently offer online pre-registration services, however you may download and print off the registration form by clicking here.

Please complete the form and bring it with you to the Surgery along with two forms of identification, one showing your proof of address and one form of photographic id.

If you are an existing patient at Feltwell Surgery, and move out of our boundary area, you will have to register with a Practice local to your new address.

Temporary Registration
If you are ill whilst away from home but need to see a GP, you can receive emergency treatment from the local Surgery for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the Practice you wish to use. Practices do not have to accept you as a temporary patient, although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Removal from the Practice List
As above, violent/abusive patients or patients who consistently DNA [do not attend] their appointments, will be removed from our Practice list.

Patients who have moved out of our Practice area will be asked to register with the Practice near to their new address.
